Parent/Guardians Meal Pick-Up/Delivery: If you provide meals to enrolled children that are staying home with their families due to COVID-19, please remember to claim the meals on the food program using the usual record keeping practice. In addition, fill out the provider verification form with parent/guardian signatures. Please click on the link below for a sample form you can use. Send this documentation to CocoKids at the end of the month for those meals to be reimbursed and make a copy for your records. This waiver remains in effect until June 30, 2020, or until expiration of the federally declared public health emergency, whichever is earlier.
